Add project notes with safe Markdown rendering.

Acceptance criteria:

• Notes persist per project with bounded input and optimistic updates.
• Markdown output allows documented safe formatting only; raw HTML/scripts cannot execute.
• HTML/JSON tests cover escaping, sanitization, missing projects, and updates.
• make check passes.
